2016 CATHOLIC PROFESSIONAL FORMATION ALLOWANCE (CPF) LEVEL 3 APPLICATION 

APPLICATIONS ARE DUE BY SATURDAY 30 APRIL 2016

The requirement for Catholic Professional Formation (CPF) Allowance level 3 eligibility is -

• the completion of one System Accredited Module (SAM) and one assessment, together with the completion of one School Accredited Module (ScAM)

or

• the completion of two System Accredited Modules (SAM) which includes the two assessments.

If you have completed a System Accredited Module which was not offered by the CEO Religious Education team you will need to forward evidence that you met the requirements of this course to Jane Knop at jane.knop@cesa.catholic.edu.au

All School Accredited Module (ScAMs) information provided will be verified by school Principals during the 2016 CPF reconciliation process.  It is highly recommended that you obtain your Principals' verbal confirmation of your ScAM completion prior to submitting this application.

Deputy Principals and Principals are not eligible for CPF Allowances. 

You will receive a confirmation email from CESA Registration when you submit a CPF3 application.  If you do not receive this email you have not successfully submitted an application.
